How Our Care Works
A clear process helps make sure care is appropriate, personalised and understood before support begins.
Contact us by telephone, email or the website. We will ask for enough information to understand the type of support being considered and whether our service is likely to be suitable.
We discuss needs, routines, strengths, preferences, communication, mobility, medication, risks, outcomes and the involvement of family or representatives where appropriate.
The agreed support is documented clearly, including what staff should do, how support should be provided, relevant risks and the outcomes the person wants to achieve.
We consider staff skills, experience, availability and compatibility, with continuity of care an important consideration.
Support starts in line with the agreed plan. Staff record relevant information and communicate changes or concerns through the appropriate process.
Care plans are reviewed and updated when needs, risks, goals or circumstances change. Feedback from the person and those involved in their care is part of the review process.
